How is your data handled on this site?

This page explains simply and literally what happens to the data you leave in the appointment request form. No complicated legal wording - only what the site really does.
What data is collected
Through the appointment request form: your name, phone number, the service you choose, your preferences about the time of the visit (free text) and your consent to data processing (a checkbox). The site collects no payment data: online consultations are paid by transfer to the doctor's card, without a payment system on the site.
Why this data is needed
Only to get in touch with you and agree on a time for your appointment. The data serves no other purpose - no mailing lists, no advertising, no passing on to third parties.
Who sees your request
Only the doctor, Ivanna Konovalets, personally. There is no reception desk, and nobody except her sees the request. No confirmation email is sent to you - the doctor answers personally by phone.
Where the data is stored
The form works through the Netlify Forms service - the technical platform the site is hosted on. The site keeps no separate database.
How long the data is stored
No longer than one year from the moment of the request. After that the data is deleted. At present the deletion is done manually; automating this process is planned as part of the site's development.

You can ask to see, correct or delete your data at any moment - write to konovaletsivanna@gmail.com.
